  • ID Copy of the Deceased

  • ID Copy of the Claimant
    The person submitting the claim must provide identification.
  • Proof of Banking Details
    This ensures that the payout is processed to the correct account.
  • BI 1663 Document (4 Pages)
    This document is issued by the mortuary and is essential for processing the claim.
